We will weigh your baby and measure their length and head circumference. We want to see that your baby has surpassed their birth weight by 2 weeks of age.

We will do a complete physical exam and answer any questions you may have. We always discuss feeding, development and safety. By 2 weeks we will have the results of the newborn screen from blood obtained in the hospital that screens for rare but serious diseases such as Sickle Cell Anemia, Cystic Fibrosis, Thyroid disease and PKU just to name a few. If there are any concerning abnormalities we will discuss these with you and formulate a plan. Usually it is just as simple as repeating the test which we can do in our clinic on the same day.  

Parent Emotions and Health Risk Questionnarie

You will also have completed an online Parent Emotions and Health Risk questionnaire prior to your visit. These will help us identify emotions parents may be experiencing due to the stressors of a newborn as well as any exposures your baby may be at risk for.

These questionnaires are recommended by the American Academy of Pediatrics and the Health Risk form is also mandated by the State of Florida for anyone receiving any government assistance with their insurance. These forms are available in English and Spanish.

We have tried to make this as easy as possible for you.

  • The answers are multiple choice.
  • All you have to do is click inside the circle of the answer you wish to choose.
  • This form may be completed on a computer, IPAD/tablet or a smart phone by accessing our website and clicking on the link to CHADIS.
  • The form is accessed electronically by Dr. Good.
  • It is scored and a report is generated.
  • If you do not have access to this technology or have trouble, we have computers in the waiting room that you may use for this purpose as well as staff to help you.
  • Please come 30 minutes early in this case to be assured you have time to complete the forms accurately and the doctor has the information for review before she sees your baby.
