****If your teenager is in 8th grade and will be transitioning to high school in the upcoming school year, please make the scheduler aware so we can schedule extra time to discuss this transition****

At this visit your teen will be weighed and their height will be measured. We will also calculate and plot their BMI. (Body Mass Index) If you are new to the practice, please see the 24 Month Well Visit above which explains the significance of BMI in children. They will have their vital signs and vision checked. We will perform a complete physical exam and answer any questions. We will discuss growth, development, nutrition, school performance and safety. We will update any vaccines. In addition, between September and March the flu vaccine will also be recommended as per the CDC (Center for Disease Control) and the AAP (American Association of Pediatrics) guidelines. Your teenager will have completed an online Pediatric Symptom Check List Youth Report (Y- PSC-17) which screens for possible cognitive, emotional or behavioral issues common to this age group as well as the Pediatric Health Questionnaire for Adolescents (PHQ-A) We have tried to make this as easy as possible for them. The answers are multiple choice so all they have to do is click inside the circle of the answer they wish to choose. These forms may be completed on a computer, IPAD/tablet or a smart phone by accessing our website and clicking on the link to CHADIS. The forms are accessed electronically by Dr. Good. The forms are scored and a report is generated for her review prior to your arrival. If your teenager does not have access to this technology or has any trouble, we will have computers in the waiting room that they may use for this purpose as well as staff to help them. Please come 30 minutes early in this case to be assured they have time to complete the forms accurately and the doctor has the information for review before the scheduled appointment time.
