At this visit you will be weighed and your height will be measured. We will also calculate and plot your BMI. (Body Mass Index) as well as take your vital signs, check your vision and hearing. If you have not had your cholesterol/lipids checked since before your 18th birthday, this appointment should be scheduled as a morning (fasting) 20 Year Old Well Visit appointment, this means nothing to eat or drink after midnight. Some patients will already have had these labs drawn, some within the last year. This is because they either had risk factors for high cholesterol or they had them drawn at their 18 or 19 Year Old Well Visits as part of the 2014 Universal Lipid screening recommendations by the American Academy of Pediatrics for preventative health care. We will obtain blood by a finger stick. The test is done in our clinic and takes 12 minutes to complete. In most cases we will discuss the results with you at the end of your appointment. A complete physical exam will be performed and any questions will be answered. Healthy living, nutrition, school/job performance and safety will be discussed. We will give you any vaccines you need including the flu vaccine between September and March. Please see our vaccine section of the website for more details. Your will have completed an online PHQ-2 as well as the CRAFFT which assesses for issues common to this age group We have tried to make this as easy as possible for you. The answers are multiple choice so all you have to do is click inside the circle of the answer you wish to choose. These forms may be completed on a computer, IPAD/tablet or a smart phone by accessing our website and clicking on the link to CHADIS. The forms are accessed electronically by Dr.Good. The forms are scored and a report is generated for her review prior to your arrival. If you do not have access to this technology or have any trouble, we will have computers in the waiting room that you may use for this purpose as well as staff to help you. Please come 30 minutes early in this case to be assured you have time to complete the forms accurately and the doctor has the information for review before the scheduled appointment time.
